Comprehensive functional mobility assessment + hands-on session

This comprehensive first session includes a functional mobility assessment to identify key areas of mechanical dysfunction and motor control deficits, in addition to a 75–90 minute hands-on manual therapy session focused on identifying and addressing lymphatic and fascial restrictions. Multiple manual therapy skills/techniques are used to re-educate the neuromuscular system and restore more efficient motor patterns in the body.

Wellness Services

Initial Visit: Private Pilates Training Session & Movement Assessment

Your initial Pilates session consists of a 5-10 minute movement assessment screen followed by your an introduction to the foundational principles and movement of Pilates.  The session will be tailored to your individual mobility and motor control/strength needs and will take place on the mat, reformer, tower, and other pieces of apparatus. 

50 Minute Follow Up Pilates Training Session

Privae Pilates training session consists of both classical and contemporary Pilates exercises with cuing that is geared to automatic core activation, postural correction, and optimal movement efficiency.   Tactile cues are provided and session is tailored to meet your mobility and motor control/strength needs.  Intro 3 Pack: $270

Gait Assessment & Coaching

Insurance FAQs

Do You Take Insurance?

FMP Physical Therapy is not a participating provider with any insurance plan.  We have found, based on experience, that one hour treatment sessions are significantly more effective then 30 or even 45 minute treatment sessions in order to provide appropriate re-assessment, manual treatment, corrective exercises, and the education that will help a patient maintain what was taught/treated in the session. We would not be able to provide the level of individualized care we provide at the rates insurance companies currently pay for physical therapy treatment. 

Will My Insurance Reimburse Me?

If you are concerned with reimbursement, we encourage you to contact your insurance company and ask about your "Out-Of-Network Physical Therapy" benefits. Make it clear that you will be providing payment at time of visit, and that you would like to know what will be reimbursed. You can often find claim forms on their website and upon request, we can provide a receipt for service with treatment codes. 


A $50 administration fee will be added if additional paperwork is required for insurance reimbursement.  



Medicare Beneficiaries

FMP  Physical Therapy is not a participating Medicare provider, and can only accept Medicare beneficiaries as patients when the patient requests that Medicare not be billed or involved in payment for their physical therapy services.  If you do  want Medicare involved in payment for your services, and have a prescription stating your physical therapy needs are medically necessary, we can recommend local clinics that participate in Medicare, but will be unable to provide you with treatment at our clinic.
