Do I need a prescription for physical therapy?
No, in the state of New York, your therapist can perform an initial evaluation without a prescription from your physician. However after 10 visits or 30 days, whichever comes first, you will be referred back to your physician to obtain a prescription, should you need additional therapy.
Do you accept insurance?
No, we do not participate directly with any insurance carriers. We recommend you contact your insurance carrier directly to see if you have "out of network" physical therapy benefits, if you are concerned with reimbursement.
What forms of payment do you accept?
We accept cash, checks made out to "FMP Physical Therapy", and credit cards. We also can accept Flex Spending or Health Savings Cards if they are affiliated with a major credit card.
What is your Cancellation Policy?
We do require 24 hour notification if you can not make your appointment. We understand that life happens, and do not charge if you have to cancel for your first time. If you have to cancel less then 24 hours more then 1x in a months' time, a cancellation fee of $50 will be charged.
